The Department of Education selects about 30% of Student Aid Reports (SARs) for verification. The Financial Aid Office may also choose a FAFSA for verification in order to resolve any questionable or conflicting information.

Please complete and submit the following by mail or fax so we may verify your FAFSA information:

A signed copy of your and your spouse's 1040 federal tax form(s)
- Both must sign
- only need 1040 or 1040EZ (do not need attachments, schedules, or addenda)--see 1040 blank example below
A signed copy of your parents' 1040 federal tax form(s) if you are a dependent student
- Dependency status is determined by your FAFSA results
- only need 1040 or 1040EZ (do not need attachments, schedules, or addenda)--see 1040 blank example below
A copy of your W2 form(s)
A copy of your spouse's/parents' W2 form(s)
The Verification Worksheet (see chart below)--must be signed.
Copies of any child support paid documentation
Copies of any other documentation that was used when filling out your FAFSA (copies of social security statement, etc. )

Please fill out forms completely and sign each of them in order for the information to be valid.
