Immunizations that are mandated by North Carolina Immunization law include:
- Two MMR immunizations (measles, mumps, rubella), with the first immunization received after the first birthday.
- Three TD or DTP immunizations (tetanus, diphtheria or diphtheria, tetanus, pertussis).
- One of the TD immunizations must be within the last 10 years.
- One PPD test (tuberculosis test) for international students or those students who have been out of the United States of America within the last twelve months. Test must have been administered within the last twelve months.
Hints on finding immunization records:
- MD office.
- High school transcript.
- Local health department.
- Grade school records.
Always send copies, never originals! All immunization records must be signed by a health professional or stamped with a medical clinic/office stamp. |
